I finally had a chance to test out a new-ish web site (JotForm.com) that allows you to add a contact form to your Facebook page. I'd say the steps are quick and easy, but that wouldn't be "the truth, the whole truth and nothing but the truth."
I consider myself a pretty advanced internet user. I know basic HTML and I've designed HTML coded web sites since the early 1990s. With that said, I have to admit that it took me awhile to figure out how to use JotForm. While it was easy to build the form, figuring out how to add it to my Facebook page was a bit of a challenge. You have to make sure you choose the Facebook option under the "Embed Form" link. If you don't use the exact code provided, your form will not work on Facebook.
So, after you create your form, choose the "Setup and Share" tab, then click on the "Embed Form" link. From there, choose the Facebook logo. The system will provideyou wit the exact code you need. Remember: you'll have to add the Static FBML application to your fan page FIRST or you won't be able to use the JotForm code provided.
